Distinction in Sports and Cultural activities: Candidates for admission to P.G. Courses / Integrated Master’s Degree Courses in Sciences, Humanities and Social Sciences, if seeking weightage for excellence in sports and cultural activities at State/ National level should fill this column and also enclose copies of certificates in support of their claim, failing which no weightage would be given.

(a) If you are in the main list of IIT-JEE 2012 or a scholar of KVPY 2012 or a participant in Science Olympiad or first ranker in CBSE/ICSE/State Boards of +2 education, provide the information and also enclose a copy of the rank card/certificate.

(b) Provide the information about the GATE subject and GATE scores if applying for M.Tech. CS/AI/IT, IC Technology, Bioinformatics and Ph.D. Electronics Science (enclose copy of the GATE score card).

(c) State whether you are a JRF/ NBHM/ICMR/ICAR/DBT / RGNF / MANF /DST INSPIRE awardee. (enclose a copy of the award letter) Candidates are required to upload the softcopy of the Photograph and Signature.

Photograph Image:

• Photograph must be a recent one and should be in passport style colour picture.
• Make sure that the picture is in colour and preferably with white background.
• If you wear spectacles then make sure that there are no reflections and your eyes are clearly visible.
• Caps/hats and dark glasses are not acceptable. Religious headgear is allowed but it must not cover your face.
• The photograph should be in 200 x 230 pixels dimensions.
• Ensure that the size of the scanned image is not more than 50KB. If the size of the file is more than 50 KB, then adjust the settings of the scanner, such as the DPI resolution, during the process of scanning.

(ii) Signature Image:

• The applicant has to sign on white paper with Black Ink/Ball point pen.
• The signature will be appended on the Hall Ticket and wherever necessary. If the Applicant’s signature on the answer script, at the time of the examination, does not match with the signature on the Hall Ticket, then the applicant will not be allowed to write the entrance examination.
• The signature should be in 140 x 60 pixels dimensions.
• Ensure that the size of the scanned image is not more than 20KB.

Submission of Application:

The candidates may submit their Hardcopy of filled-in online application either by post addressed to the Deputy Registrar (Acad & Exams), University of Hyderabad Campus, Hyderabad 500 046 or in person at the Academic Section, in the office of the Controller of Examinations, University of Hyderabad Campus, Hyderabad.

The application and other relevant papers are to be pinned/stapled in the following order:

i. Hardcopy of the filled in online application form duly affixing a passport size photo at the space provided (Photo should be affixed using the gum but should not be stapled)

ii. Bank Demand Draft / Challan

iii. Attested copy of the latest relevant certificate in respect of SC/ST/OBC/PWD/DP candidates

iv. Attested copy of the GATE / JRF, in the case of applicants for M.Tech / M.Phil/ Ph.D.

v. Attested copies of the certificates in support of distinction achieved in sports and cultural activities at State / National level. (if applying for admission to P.G. / 5-year Integrated P.G. courses only)

vii. Attested copies of (a) Rank Card of IIT-JEE 2012 or the award letter of the KVPY 2012 or rank certificate of +2 education or the participation certificate in Science Olympiad 2012 (b) Medium of Instruction at +2 level and (c) Proof of Residence & Study Certificate upto +2 level for Backward District candidates in the case of candidates applying for M.A./M.Sc. (5-year Integrated) Courses.

Incomplete applications, applications not accompanied with the requisite enclosures and those received after the closing date will not be entertained.
